🫱 Before the Appointment - You’ll receive a warm welcome via email or phone confirming the time - We’ll ask if you'd like the appointment in person, via phone, or video call — whichever suits you best - You’ll be invited to bring your NDIS plan, questions, and anyone important to your care journey (like a family member or carer) 🗣️ During the Appointment What We Cover: - Introductions & getting to know you - Review of your NDIS goals, funding, and existing supports - Listening to your story: what’s working, what’s challenging, what you hope for - Explaining what support coordination means in simple terms - Mapping out next steps: referrals, service options, timelines, and how we’ll work together 💙 After the Appointment - You’ll receive a follow-up summary outlining what we discussed - If you choose to move forward, we’ll prepare a service agreement tailored to your needs - You’ll have a dedicated coordinator (like you’ve got a care teammate!) — ready to begin walking beside you
